What To Expect on Your First Visit

We will ask you to complete paperwork such as a health history form, HIPAA form, consent to treat form, insurance card and insurance information.  Please alert the office if you have a medical condition that may be of concern to us such as diabetes, high blood pressure, artificial heart valves or joints, etc.  Also, please inform us if you require premedication prior to dental cleanings.  Please be patient since once this is on file, we will only need to do simple periodic updates.

To expedite the necessary paperwork at your first visit, you can download and fill out the forms at home and bring them to your visit.  These can be found on the page entitled "Contact and Forms."  You may wonder why we ask certain questions and the reason is that dental health is closely related to overall health.  There are medications that can interact with each other or with dental anesthetic.  Some medications can cause dental problems such as dry mouth and cavities.

We will take necessary digital radiographs and the Dr. will perform a thorough dental examination and oral cancer screening. An intraoral camera may be utilized to show you "your mouth." At that time, diagnoses will be determined and a treatment plan with options will be discussed. Fees will be determined and a future appointment (if needed for treatment) may be made in the treatment room. Occasionally, treatment can be done the same day. Any questions you have can be discussed at that time as well. If you are scheduled for an examination and a cleaning, the cleaning will be done after the exam. We have large screen LCD monitors in each treatment room so that you may view your radiographs and intraoral video of your teeth, if so interested. If your previous dentist has taken recent x-rays (within 6 months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional radiographs are necessary, they can be taken digitally at our facility.
